About the Opportunity
As a Clinical Care Coordinator, you’ll play a key role in the leadership team of an aged care facility. Reporting to the Care Manager or Director of Nursing (depending on the site), you will:
- Provide clinical leadership and mentoring to the care and nursing teams
- Oversee the delivery of complex and high-impact care
- Ensure compliance with Aged Care Quality Standards and care governance
frameworks - Drive continuous improvement in clinical outcomes
- Support education and professional development within the team
- Build strong relationships with residents, families, and healthcare partners
You’ll work in a collaborative environment where your input is valued and where growth opportunities into senior clinical or management roles are actively supported.
Key Responsibilities

Coordinate and oversee safe, high-quality nursing care aligned with residents’ goals

Lead care planning, assessments, and high-prevalence risk management

Supervise clinical staff and support the development of team capability

Support accreditation processes and incident reporting

Contribute to workforce planning and clinical governance initiatives

Champion a positive workplace culture that puts residents first
About You
We are seeking an individual who combines expertise and passion for aged care leadership.
You will need:
- Current AHPRA registration as a Registered Nurse
- Minimum 2–3 years of clinical aged care experience (leadership experience highly regarded)
- Strong understanding of the Aged Care Quality Standards
- Demonstrated leadership, mentoring, and coordination skills
- Excellent communication and interpersonal abilities
- Sound computer literacy and experience with clinical documentation systems
- Postgraduate qualifications in aged care, leadership, or management (desirable)
- Commitment to ongoing professional development
Additional requirements:
Valid working visa or Australian residency
Current Police Check (or willingness to obtain)
Up-to-date Influenza and COVID-19 vaccinations
