Inadequate wound management is one of the most common clinical complaints in the aged and social care sector. Common mistakes can include infrequent wound assessments, failing to document key information (such as wound dimensions and the amount and type of discharge) and failing to re-assess the wound when it deteriorates. Effective wound management is essential in preventing complications and ensuring optimal healing. A comprehensive wound assessment is fundamental to this process. All staff providing wound care need to understand how to conduct an initial and ongoing wound assessment and what action is required when signs of infection or deterioration are identified.
